Preliminaries: History, Exam Strategy, and Office Drugs KEY POINTS • Treat every patient with respect. Some are very specific for a particular class of organisms; others are broad-spectrum (ie, effective against a wide variety of bacteria). Therefore, it is important to take cultures before treatment has begun. Some severe ocular infections may require systemic medication. Patients should be instructed to adhere to the dosage regimen; prolonged use or overuse of antibiotics may lead to the development of bacteria that are resistant to the drug. Antifungal Drugs Ocular infections caused by fungal organisms are less common than other infections but can have devastating effects on the cornea or intraocular structures. They are indicated for external or intraocular inflammation caused by allergies, infections, toxic reactions, or immune reactions (such as corneal graft rejection). They are also very effective against the intraocular inflammation associated with auto-immune disorders, such as rheumatoid arthritis or lupus, and may be administered topically, systemically, or by periocular injection.
